About the Role

Join the team as an Elder Care Connector, supporting Aboriginal clients aged 50+ to access aged care services. Using a strengths-based, culturally respectful approach, you’ll engage with clients, families, and carers, providing advocacy, care navigation, and liaison with service providers. You’ll support allied health assessments, maintain program documentation, and contribute to resource development, promotional activities, and ongoing professional development. This hands-on role delivers meaningful impact and aligns fully with cultural safety and organisational values.

Experience / Requirements

• Qualified Social Worker with aged care or community experience
• Familiarity with MyAgedCare desirable
• Strong understanding of Aboriginal cultural values and self-determination
• Willingness to travel to remote areas via car or light aircraft
• Experience engaging with Aboriginal communities in a culturally safe way
